Affected individuals who experienced extraordinary losses due to the data breach, including identity theft and fraud, can also receive up to $5,000 in compensation.
Class members are eligible to receive two years of free credit monitoring services – including single-bureau monitoring and at least $1 million in fraud insurance.
People who chose to receive a cash payment instead of credit monitoring can get their hands on a cash payment of an estimated $45.
This amount is subject to change depending on the number of claims filed with the settlement.
The compensation covers any losses including fraudulent charges, identity theft expenses, credit monitoring costs and more.
Affected individuals will need to provide documentation of their loss.
This could include receipts, bank statements, credit card statements, invoices, bills and more.
Class members have until January 12 to make a valid claim.
This comes after two partnering health care providers were accused of a similar data breach that left patients vulnerable to fraud.
